PelviZen LLC provides a digital platform that includes a diagnosis discovery algorithm designed to provide personalized pelvic health recommendations. Formal telehealth consultations are facilitated via a third-party Electronic Medical Record (EMR) provider, Tebra Health.

3. Use of Discovery Algorithm (Educational Disclaimer)

The "Diagnosis Discovery Algorithm" on our website/app is a tool to help guide your care.

• Informational Purposes: The algorithm provides suggestions based on your input; it does not constitute a final medical diagnosis or a physical examination.
• Clinical Validation: Final clinical diagnoses are only provided during your direct telehealth consultation with a licensed provider via Tebra Health.

4. Third-Party Providers

By using our services, you acknowledge that certain functions are handled by third-party partners:

• Telehealth/EMR: Clinical care is managed through Tebra Health. You agree to abide by Tebra's terms and privacy protocols during your consultation.
• Payments: All billing is processed through Stripe. PelviZen LLC does not store your credit card information.

5. User Responsibilities

You represent and warrant that:

• You are at least 18 years of age.
• The information you provide to the algorithm and our staff is accurate and truthful.
• You will not use the service for any fraudulent or illegal purposes.

6. Intellectual Property

The algorithm, website design, logo, and content are the exclusive property of PelviZen LLC. You may not copy, reverse-engineer, or distribute any part of our algorithm or proprietary treatment recommendations without written consent.

7. Limitation of Liability

To the maximum extent permitted by law, PelviZen LLC and its affiliates shall not be liable for any indirect, incidental, or consequential damages resulting from your use of the website or algorithm. While we strive for accuracy, we do not guarantee that the algorithm's recommendations will be 100% accurate for every individual.

8. Termination of Service

We reserve the right to terminate or suspend your access to our services at our sole discretion, without notice, for conduct that we believe violates these Terms or is harmful to other users or our business interests.

9. Governing Law

These Terms are governed by the laws of the State of Oregon, without regard to its conflict of law principles.
